Mosaic Wellness & Recovery and its affiliated entities (“Mosaic,” “we,” “us,” “company,” “facility,” or “our”) value the privacy of everyone who visits our website, contacts our centers, or otherwise interacts digitally with our company or services (“programs,” “brand,” or “tracks”). This Online Privacy Policy explains what information we gather, how we use and share it, and the choices available to you. By using our Services, you agree to the practices described below.

INFORMATION WE COLLECT: We gather information in two main ways: what you choose to share with us, and what our systems collect automatically as you interact with our Brands.
INFORMATION YOU GIVE US DIRECTLY: When you fill out a contact form, submit a job application, request information, or otherwise reach out to us through the Services, we may ask for details such as your name, mailing address, email address, and phone number. We only receive this kind of information when you decide to provide it – it isn’t collected without your action.
INFORMATION COLLECTED AUTOMATICALLY:
DEVICE AND BROWSING DATA: Our servers and analytics tools may log technical details each time you visit, including your IP address, approximate geographic location, device and browser type, referring pages, the pages you view, and timestamps of your activity.
COOKIES AND SIMILAR TOOLS: We may use cookies, pixels, and comparable tracking technologies to recognize returning visitors, remember preferences, and understand how the Services are used. Most browsers let you limit or block cookies through their settings.
ANALYTICS AND ADVERTISING PARTNERS: We may work with outside analytics and advertising vendors who use their own tracking technologies to help us understand usage patterns and, where applicable, deliver relevant ads on other sites. We use Google Analytics for this purpose; it does not provide us with information that personally identifies you, and Google’s handling of that data is governed by Google’s own privacy policy.
INFORMATION FROM OTHER SOURCES: We may also receive information about your interactions with our social media pages or from third parties who already had your information and chose to share it with us.
HOW WE USE AND SHARE INFORMATION: We may use the information we collect to respond to your inquiries, send newsletters or updates you’ve requested, evaluate job applications, monitor and improve the security and performance of our Services, detect and prevent fraud or misuse, and generally support our operations.
We do not sell your personal information for money. We may, however, share information in these circumstances:
With vendors and service providers who perform functions on our behalf, such as web hosting;
If our organization undergoes a merger, acquisition, financing, or sale of assets;
With your consent or at your direction;
In response to a subpoena, court order, or other lawful government or legal request;

We may also share information that has been aggregated or stripped of identifying details, which we use to better understand and improve our Services.
LINKS TO OTHER WEBSITES: Our Services may link out to websites we don’t control. We aren’t responsible for the privacy practices of those third-party sites, so we encourage you to review their policies before sharing any information with them.
HOW WE PROTECT AND RETAIN INFORMATION: We use reasonable administrative, technical, and physical safeguards designed to protect the information we collect. That said, no method of transmitting or storing data online can be guaranteed to be completely secure, so any information you send us is sent at your own risk. We retain personal information only as long as reasonably necessary for the purposes described in this policy or as required by law.
DO NOT TRACK: Because there’s no industry-wide agreement on how “Do Not Track” browser signals should be interpreted, our Services currently do not respond to them.
CHILDREN’S PRIVACY: Our Services are not directed to, and we do not knowingly collect personal information from, children under 18 without verified parental consent. If you are under 18, please refrain from submitting personal information through our Services.
NOTICE FOR CALIFORNIA RESIDENTS: If you’re a California resident, the California Consumer Privacy Act (CCPA) gives you certain additional rights regarding your personal information, summarized below. This section doesn’t apply to information already regulated separately, such as protected health information under HIPAA or the California Confidentiality of Medical Information Act, or information covered by other sector-specific laws like the Fair Credit Reporting Act or Gramm-Leach-Bliley Act.
YOUR RIGHTS UNDER THE CCPA INCLUDE:
Right to Know: You may ask what categories of personal information we’ve collected, where it came from, why we collected it, and who we’ve shared it with.
Right to Deletion: You may ask us to delete personal information we hold about you, subject to standard legal exceptions (for example, where we need it to complete a transaction, comply with a legal obligation, or detect fraud/security incidents).
Right to Opt Out of Sale: You may ask whether we’ve sold your personal information in the prior 12 months and opt out if applicable.
Right to Non-Discrimination: We won’t penalize you for exercising these rights, though certain services may not function properly if you delete information necessary to provide them.
Categories of personal information we may have collected in the past 12 months include identifiers (like name and email), customer records information, protected classification information, commercial information, internet/network activity, geolocation data, and inference-based profile information – generally corresponding to what’s described in Section 1 above.
To submit a request under the CCPA, or to designate an authorized agent to act on your behalf, please contact us using the information in the “Contact Us” section below. We may need to verify your identity before completing your request, and parents/guardians may submit requests on behalf of a minor.
UPDATES TO THIS POLICY: We may update this Privacy Policy periodically to reflect new features, legal requirements, or changes in our practices. Updates take effect as soon as they’re posted, and the “Last Updated” date at the top will reflect the most recent revision.
