Insurance & Cost
Mosaic works with most major insurance providers including Aetna, Blue Cross Blue Shield, Cigna, Humana, and UnitedHealthcare, as well as UMR, Optum, and many employer-sponsored plans. If you’re unsure whether your plan is accepted, call us and we’ll verify your benefits at no cost before you make any decisions.
The cost of treatment depends on your insurance coverage, the level of care recommended, and the length of your stay. We provide a clear, honest breakdown of expected costs — including deductibles, copays, and any out-of-pocket responsibility — before treatment begins. What’s covered depends on your specific plan, the level of care recommended, and whether prior authorization is required. The Admissions Process
Getting started is as simple as contacting our admissions team via phone, web form, or online chat. From there, we’ll answer your questions, verify your insurance, conduct a clinical assessment, and guide you through every step. Most clients are admitted within 24 to 48 hours of their first conversation with us.
Our clinical team conducts a comprehensive intake assessment to understand your history, current symptoms, substance use, mental health concerns, and personal goals. This assessment helps us identify the most appropriate level of care and build an individualized treatment plan around your specific needs. It’s a conversation, not an interrogation — you’ll be treated with respect and without judgment throughout.
Most guests are admitted within 24 to 48 hours of their initial call. In some cases, same-day admissions may be possible depending on availability and clinical appropriateness. Our admissions team moves quickly because we understand that timing matters.
No referral is required to contact our admissions team or begin the intake process. If your insurance plan requires a referral for certain levels of care, our team will help identify that early and guide you through the appropriate steps.
Treatment & Programming
You don’t have to figure that out on your own — that’s what our clinical assessment is for. Our licensed clinical team evaluates your history, the severity of your symptoms, your safety, your support system, and your treatment goals to recommend the most appropriate level of care. Options include Residential Treatment, Partial Hospitalization (PHP), Intensive Outpatient (IOP), Evening IOP, and Outpatient services.
Length of treatment varies by individual and level of care. Residential programs typically range from 30 to 90 days depending on clinical progress and plan coverage. Outpatient programs (PHP, IOP) are structured around program schedules and individual progress. Your treatment team will discuss length of stay recommendations with you during the admissions process and will continue to evaluate progress throughout your time at Mosaic.
Yes. Dual diagnosis — co-occurring mental health and substance use disorders — is a core specialty at Mosaic. We treat both conditions simultaneously rather than sequentially, because addressing only one often leaves the root causes of the other unresolved. If you’re experiencing depression, anxiety, trauma, PTSD, or another mental health condition alongside substance use, you’re in the right place.
Mosaic uses a range of evidence-based and holistic modalities tailored to each individual’s treatment plan. These include Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), Acceptance and Commitment Therapy (ACT), Eye Movement Desensitization and Reprocessing (EMDR), Motivational Interviewing (MI), Internal Family Systems (IFS), Medication-Assisted Treatment (MAT) where appropriate, mindfulness, breathwork, and experiential approaches.
Daily Life & Logistics
Our admissions team will provide a detailed packing list when you are confirmed for admission. Generally, you’ll want to bring comfortable clothing, personal hygiene items, any prescribed medications in their original containers, a government-issued photo ID, and your insurance card. Items like electronics, certain supplements, and specific personal items may have guidelines depending on the level of care — our team will walk you through everything in advance.
Yes. Family involvement is encouraged as part of the healing process. Mosaic offers family therapy and education sessions designed to rebuild communication, address relational dynamics, and strengthen the support system that matters most to your long-term recovery. Your treatment team will discuss how and when family involvement is most appropriate for your specific situation.
Discharge planning begins early — not the week before you leave. Our clinical team works with you throughout treatment to build a comprehensive aftercare plan that may include step-down to a lower level of care, outpatient therapy, peer support, community resources, and relapse prevention strategies. Our goal is to make sure you leave with a clear, practical plan for what comes next — not just a certificate of completion.
